First Visit to Sage Spa

Welcome! For your safety, you will be asked to complete a medical intake form upon arriving. You can also download the form here, print it, and complete it at home to bring with you. Please arrive at least 15 minutes before your appointment time. We don’t want you to feel rushed and stressed before your treatment! We all want your visit to be perfect!​

​Cancellation Policy

We understand that your activities are not fixed, and your schedule can change. We request that you inform us of your plans to cancel or reschedule your appointment at least 24 hours before your appointment. Appointments canceled within 24 hours will incur a 50% charge of the service fee.

Gift Certificate Policy

Gift cards are sold in the spa or online in any amount.  Gift cards are not refundable or redeemable for cash. All gift certificates are non-refundable and cannot be replaced if lost or stolen.

Late Arrival for Appointment

If you are late for an appointment, your treatment will end on time so that the next appointment may begin as scheduled. Should this happen, your service will be charged in full.

Minors

If you are not yet 18 years old, you will need to have a parent or legal guardian sign your Intake Form to authorize your treatment. If the minor is over 16, parents do not need to remain present during the minor’s treatment, however, they are welcome to do so. Minors under the age of 16 will be required to have a parent in the treatment room.

Gratuity

Gratuities are accepted and appreciated.

Right to Refuse Service

We reserve the right to refuse or discontinue service to anyone demonstrating behavior that is perceived to be inappropriate or disruptive to our Spa atmosphere.

Prices and services are subject to change at any time.

Payment/Deposits

We accept all major credit cards, cash, and Apple, Google, and Cash App Pay. A credit card is required to hold all appointments. 50% deposit is required for all services 2 hours or longer.
